- "76/ W T/ P/ B" stamped into base, near rod. "1036" and "116/36" crudely incised on base/ UHN catalogue no. "994.1.27" printed by hand in white ink on side panel of base. "REP'D/ SEP. 1963" printed on tape label on underside of base. Graphs on paper chart labelled "arterial pressure/ 100mm./ 50mm./ 0mm." , " E.C.G./ /___/ 1 sec.", "[up arrow]/ stimulator on/ [down arrow]", and "[up arrow]/ stimulator off/ [down arrow]".

- Unknown- appears complete. One foot on base may be replacement. Paper chart is replacement/reproduction.
